Submission Requirements

In order to properly classify, price, and underwrite an account the following information is required. This will enable us to more quickly quote your account and provide the best possible insurance program for your client.
• Narrative of the insured's operations
• Completed Acord application for each line of business with details on all Named Insureds and Additional Named Insureds
• Fairmont Specialtyฎ Supplemental Application(s)
• 4-year currently valued loss runs with details on any claim greater than $25,000
• Statement of Values for Property
• Complete Auto schedule with VIN numbers and cost new
• Drivers list with DL # , Date of Birth, and Date of Hire
• Copy of any contracts containing special liability agreements
• Schedule of Mobile Equipment
• Business Income Worksheet

In some cases, based on the account's operations, more than one supplemental application may be required.
